To be completed by all new and returning clients before every appointment.

To be completed prior to any massage service including Swedish massage, Deep tissue and Aromatherapy massage. (Please complete a Pregnancy Consultation when having a Pregnancy Massage) 

To be completed prior all tinting treatments. 

To be competed before any waxing consultation. 

For any clients under the age of 18 years old a parental consent form must be compete by the parent / guardian of the minor prior to any treatment/s being carried out.

To be completed prior to any pregnancy massage. Please be advised you must be 13 weeks or over to have a pregnancy massage and have a letter from your health care provider if you are a high risk pregnancy. 

A Patch Test Consent form must be completed at least 24 hours before your first treatment and repeated every 6 months.
